Long Term Disability

At no cost to you, a Long Term Disability plan.   This plan will provide you with 60% of your covered monthly earnings to a maximum benefit of $5,000 per month.

Short Term Disability

At no cost to you, a Short Term Disability plan.  This plan will provide you with 60% of your covered weekly earnings to a maximum benefit of $500 per week.

Term Life & Accidental Death & Dismemberment

At no cost to you, you have coverage in the amount of 1 X Annual salary with a minimum of $25,000  (i.e. Your annual income is $18,000; your benefit is $25,000.  Your annual  income is $30,000; your benefit is $30,000) and a maximum of $75,000.

The Standard Voluntary Life Insurance

You have the opportunity through payroll deduction, to purchase additional Term Life insurance that can supplement the employer paid life coverage.  The cost is based on your age and your level of coverage selected.

 

Vacation

Two (2) weeks after one (1) year of employment and 3 weeks after 6 years of employment.
You can cash one week out, but you must take the other week vacation.

  

401 (K) Plan

You are eligible when you have completed 1 year of service, 1000 service hours and are 21 years of age. You may elect to defer up to 15% of your gross earnings on a bi-weekly basis and the company will match 100% of your contributions up to 3%. Associates become 100% vested in the 401(K) plan after 6 years of continuous employment.
